Understanding value-based care and population health management continues to be an imperative as the drive towards high quality healthcare gains momentum as CMS looks to increasingly tie payments to quality and outcomes versus quantity of services. Join this session to learn how the quality awards and Baldrige criteria align with long term care focused population health management strategies such as institutional special needs plans (I-SNPs) and accountable care organizations (ACOs).

Nurse Learner Outcome: At the conclusion of this session, participants will self-report a knowledge gain how the quality award program aligns with value based care practices.
