Description

The goal of person-centered care is to guide care and to recognize and honor resident goals, preferences, and values. Success is measured by patient-reported outcomes by providing coordinated care, managing chronic and complex conditions and building trusting relationships with caregivers. That said, care decisions made by families and residents can increase risk to your community. This program was developed to assist your team in identifying when risk outweighs the benefit of person-centered care and how to address these risks. Case studies will be discussed along with strategies to decrease risk while providing care.

  1. Identify the key elements of person-centered care.
  2. Describe the risks and benefits of person-centered care.
  3. Define the process of managing difficult situations that may arise with person-centered/person directed care.
