The payer landscape in long-term care is rapidly shifting, and Institutional Special Needs Plans (I-SNPs) are at the center of that change. This session will explore how I-SNPs support improved resident outcomes while helping skilled nursing facilities succeed financially. Participants will learn how these plans emphasize prevention, reduce costly rehospitalizations, and integrate nurse practitioners directly into care teams to support residents’ long-term health. For operators and clinicians alike, this session will highlight why I-SNPs are becoming the future of post-acute care—and how to position your facility for success in this evolving environment.

Nurse Learner Outcome: At the conclusion of this session 80% of the participants will self report a knowledge gain in how Nurses can work with the ISNP program to improve the patients experience in long term care.
