Heather Despres Cook obtained a master’s degree in physical therapy and currently in pursuit of a master’s degree in health informatics. Heather’s career tenure includes over 15 years in post-acute care in various clinical and leadership roles. In her current role as Value-Based Care Navigator at Corewell Health, she has been instrumental in mentoring, influencing, and leading valued-based care for beneficiaries of the West Michigan Accountable Care Organization and Corewell Health’s High Performing Network of skilled nursing facilities and home health agencies. Heather has been the subject matter expert regarding data analytics for the High Performing Network.
