Session Date: Thursday, November 14, 2024

Session Time: 2:00 PM - 3:00 PM (UTC-04:00) Eastern Time (US & Canada)

This program will be held live on Zoom. No CE will be offered for on-demand participation, although the session will be recorded for later access.

Is your facility looking to improve your quality outcomes? HCAM is partnering with Corewell Health to offer a webinar that will show how skilled nursing facilities and hospitals can share cost and quality care data to provide value-based care. Corewell Health’s Value Based Care Navigator Heather Despres Cook and West Michigan Accountable Care Organization Executive Director Andrea Charmley will share methods that nursing facilities can use to leverage relationships and improve communications to provide increased care. Administrators, CEOS, regional directors and other leaders are encouraged to attend. The webinar will also address ways to build your business through partnerships and provide success stories.

Learner Objectives:

  1.     Discuss how both the hospital and the SNF benefit from high performing network arrangement for quality patient care.
  2.     Examine the correlation of hospital and SNF relationships to value-based care success.
  3.     Evaluate future collaboration and program opportunities to enhance partnerships between hospitals and SNFs.
