This program will be held on Zoom. The session will be recorded and available on-demand, however, no CE will be offered for on-demand participation.

 

Do you have a facility acquired pressure injury or more than one? Did you complete a root cause analysis, PNC, and review it in your QAPI meeting? These are three important things that must be completed for process improvement, to ensure quality care is provided and because a surveyor will ask for them! This session will teach you how to complete a thorough root cause, analysis, PNC, and what the QAPI process is and the importance of it is. All in an effort to reduce the development of facility acquired pressure injuries!

Learner Objectives

  1. Identify the key components of Root Cause Analysis (RCA), Plan of Correction (PNC), and the QAPI process related to facility-acquired pressure injuries.
  2. Explain the steps required to complete a Root Cause Analysis following the development or worsening of a facility-acquired wound.
  3. Apply the process for developing a Plan of Correction (PNC) based on findings identified through Root Cause Analysis.
  4. Analyze how Root Cause Analysis and PNC findings integrate into the QAPI process to support quality improvement and regulatory compliance.
  5. Evaluate opportunities for process improvement within their facility to reduce the occurrence of facility-acquired pressure injuries.

Nurse Learner Outcome: At the conclusion of this session, 80% of participants will self-report an increase in knowledge related to conducting root cause analyses, developing plans of correction, and incorporating findings into the QAPI process to improve pressure injury prevention and wound care outcomes, as measured by the post-session evaluation.

RN: This program will provide participants the opportunity to earn up to 3 nurse contact hours. Health Care Association of Michigan is approved as a provider of nursing continuing professional development by the Wisconsin Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation. This program is jointly provided with MI-NADONA/LTC.

NHA: The Health Care Association of Michigan is a Certified Sponsor of continuing education with the National Association of Long Term Care Administrator Boards (NAB).

  • This program has been submitted, but not yet approved for Continuing Education for 3 total participant hours by NAB/NCERS - Approval #
  • NHA CE is not being offered for on-demand completion of this program.

CE Reporting:  HCAM will need 30 days after the event conclusion to compile and process all attendance data. Once the data is compiled, certificates will be available to access. Certificate questions should be directed to Mary Gettel.

Requirements of Participation:

  1. Attendance is being strictly monitored.
  2. Be sure you are entering and exiting sessions at the start and end of the session – if you miss more than 10 minutes because you arrived late or left early, you will lose partial credit. HCAM will issue partial credit by half hour attended. It is YOUR responsibility to follow the agenda and be on time!
  3. A post-test must be successfully completed with a passing score of 85% or higher in order to earn credit.
  4. The evaluation must be completed in order to earn your certificate of completion.
  5. Attendees caught cheating or violating our attendance policies will not receive credit for the remainder of the event.
